Therapeutic Efficacy of Intratympanic Steroid Injection Accompanying Combination Therapy in the Patients with Sudden Sensorineural Hearing Loss£ºAnalysis According to Injected Drug

Abstract
Background and Objectives : Intratympanic administration of steroid for the patients with sudden sensorineural hearing loss (SSNHL) has been reported with the results of significant improvement of hearing as a initial therapeutic modality or salvage method. We performed a prospective study to evaluate the therapeutic efficacy of two types of intratympanic steroid injection accompanying combination therapy with systemic steroid in the patients with SSNHL.

Materials and Method : Patients with SSNHL who received combination therapy with systemic steroid and concomittant intratympanic steroid injection between April 2004 and July 2006 were included in this study. According to the type of intratympanically injected drugs, they were divided into group 1 of 29 patients (dexamethasone injected group) and group 2 of 35 patients (methylprednisolone injected group). The recovery rate, changes of hearing levels, speech discrimination scores, side effects and complications of intratympanic steroid injection were evaluated.

Results: Known prognostic factors for hearing recovery in SSNHL including initial hearing levels, age, presence of vertigo, shape of audiogram and intervals between onset of SSNHL and treatment were not significantly different between group 1 and 2. Overall recovery rate of group 1 was 51.7% and that of group 2 was 77.1% and the difference was statistically significant (p<0.05). Significantly higher recovery rate was observed in group 2 especially in the patients with age under 60 and with non-ascending type of hearing loss. Otalgia scores in group 2 was significantly higher than in group 1.

Conclusion: Intratympanic methylprednisolone injection accompanying combination therapy with systemic steroid administration in the patients with SSNHL might be more effective than intratympanic dexamethasone injection. Consideration of a good method to decrease otalgia after intratympanic methylprednisolone injection and the further studies with larger population will be necessary).
